APPENDIX 3 - EMPLOYER FOLLOW-UP LETTER

Dear Employer,

 

RE:  DAVE SMITH, Work Experience participant.

 

On June 25, 2002, a letter of introduction and response card were sent to you regarding employer satisfaction with specific programs.  The Employment Resources Division of the Wyoming Department of Employment is required by law (Workforce Investment Act, P.L. 1005-220) to gather customer satisfaction information about the Work Experience, On the Job Training, or Rapid Response program services in which your business recently participated.

 

If you still have the original materials in your possession, please return the address card so that we may perform a brief telephone survey.  On the address card, we ask that you provide a telephone number where you can be reached, and the best time to contact you during regular working hours.  If these times are not good for you, we can make accommodations to call you in the evening or during the weekend.

 

Alternatively, complete the enclosed form and return it using the accompanying business reply envelope.  Please mail the completed materials by July 26, 2002.  If you have returned the original card, no further action on your part is necessary; we will contact you per the given responses.
