Dear BOB SMITH,

                                                                            

The Employment Resources Division of the Wyoming Department of Employment is required by law (Workforce Investment Act, P.L. 1005-220) to gather customer satisfaction information about the services you recently received from your local Employment Center.  This constitutes part of the follow-up that you agreed to as part of the services in which you participated.

 

Research & Planning, a separate section in The Department of Employment, has been asked to collect customer satisfaction information in a phone interview.  The interview involves only four questions and should take no longer than 10 minutes.  Your answers to these brief questions will assist the Employment Resources Division in improving the quality of services that it provides to future program participants, like yourself.  As such, your views are critical.

 

Research & Planning is connected with the Employment Resources Division only for contracted research functions.  Results of this study will only be made available outside Research & Planning in the form of a statistical report.  Your responses and any identifying information about you or your employer is protected by Wyoming State Statute (WS 27-3-603) and will be held confidential within the Research Section to the extent permitted by law.  Further, your participation in this survey will in no way adversely affect access to future services from the Employment Resources Division of the Wyoming Department of Employment. 

 

Enclosed you will find an addressed card for you to mail back to Research & Planning.  We ask that you provide a phone number where you can be reached, and the best time to contact you during regular working hours.  If these times are not good for you, we can make accommodations to call you in the evening or during the weekend.  If you indicate that a phone interview is not possible, a paper survey will be mailed to you to fill out and return to Research & Planning.  Please send the completed postcard by April 22, 2002.
